We are pleased to announce that Portsmouth Regional Hospital has launched a new program called Care Assure! This program is designed to identify patients with Cardiovascular Disease or those at risk and require follow up care after an in-patient or observation stay.
Portsmouth Regional Hospital has assigned a nurse to serve as a Care Assure Nurse Navigator. The navigator will review patient records and physician follow up instructions to ensure that the patient completes their follow up visits as determined by their attending physician.
For patients who are actively followed by a PCP or Cardiologist, the Nurse Navigator will work with the patient to schedule follow-up appointments with their physician and confirm that they have attended the appointments. They will also answer questions and provide education to the patients regarding care for their condition. The program currently will focus on patients with valvular disease, arrhythmias, reduced ejection fraction, and patients in need of chronic cardiovascular disease management.
