Release of Medical Record Information
Please be advised that all requests for medical record information are processed and released by HealthPort Technologies.HealthPort is a copy service that HCA has partnered with to provide release of information services for all HCA hospitals.
Effective December 31, 2011, all medical record requests for Portsmouth Regional Hospital will be forwarded to the Health Information Management Shared Services Center for processing by HealthPort.
In order to better serve you and avoid processing delays, please complete all sections of the release of information authorization form. Requested information will be mailed to you from the HealthPort Corporate office withinthree to five business days.
The cost of copies will be dependent on the information requested. Copy charges include .25/page and $1.00/page for microfilm. An invoice will be provided with your copies.
