Wellness Connection

Healthcare is about much more than just advanced technologies and dramatic surgical procedures. Ultimately, it's about helping you achieve the best quality of life possible, and PRH's Wellness Connection was established to do just that. The Wellness Connection offers an array of assessment, education, exercise and rehabilitation services to help you stay well, manage existing health conditions and prevent new problems.

Participation begins with a Wellness Profile assessment that identifies your health needs and creates an individualized wellness program for you. Education, exercise and rehabilitation programs are other important parts of the wellness equation, helping you change your lifestyle and live in a healthier way. These are implemented through the Center's Disease Management Services, Primary Prevention Services, and Comprehensive Services.

Wellness Connection education programs concentrate on such problems as heart disease, emphysema, and diabetes. Major areas of emphasis are cardiac rehabilitation, pulmonary rehabilitation, and women's programs on issues ranging from breast cancer to menopause. Lifestyle and nutrition counseling is offered, as are courses in smoking cessation, stress management and relaxation techniques, yoga, massage therapy, and weight management. Supervised exercise programs, including one for children, are part of the offerings.

Health Assessment

To identify health issues before they become health problems, every individual who enters a Wellness Connection program is given a Wellness Profile, which involves an assessment of health behaviors, blood pressure and cholesterol checks and recommendations for improving healthy behaviors. Each individual's needs are addressed on an individualized basis.

Disease Management

The Wellness Connection Disease Management Service focuses on chronic diseases and people who are at risk for disease. We recommend lifestyle changes known to reduce those risks and assist you in making healthier choices before problems occur.
