Intensive, Patient-Centered Treatment Process
For most patients, treatment involves weekly visits for 2 to 2 1/2 months. During the first three visits, we work to understand what has caused the wound and prevented it from healing. The initial consult involves a 1 1/2 - 2 hour visit during which we conduct a complete history and physical. Diagnostic tests are ordered for that same day or within the following week. The results of these visits help us decide which specialist should be consulted.
By the third visit, after the patient has met with the appropriate specialist, the Wound Care Center team knows what's going on and why the wound won't heal. We create a treatment plan designed to address the cause as well as the wound itself.
During the first 4-6 weeks, the patient usually comes once a week. During each visit the patient works with the same team members establishing a warm and caring relationship that greatly enhances the healing process and helps patients stick with the program.
Treatment may consist of all or some of the following: debridement, infection control, revascularization, skin grafting, compression therapy and hyperbaric medicine. Some patients benefit from surgery to repair and reconstruct damaged bones and ligaments.
Later on in the course of treatment the patient may see an additional Wound Care Center specialist. Throughout the treatment program, every member of the Wound Care Center team works together within a customized plan of treatment, communicating and coordinating each step in the process. This integrated approach results in the fastest possible healing.
Throughout the treatment program, our nurses provide patient education about wound care, nutrition, exercise and prevention. We also stress communication and followup with the patient's primary care physician. This followup is vital to the prevention of future chronic wounds.
Vascular surgery, laboratory, radiology, skin grafting, home health care, pharmacy, nutrition, infection control, protective devices, rehabilitation and patient education are all approaches our specialists may use to treat a wound. The physicians and nurses of the Wound Care Center provide education to help patients care for existing wounds as well as prevent future wounds.
Outstanding Outcomes
Since the Wound Care Center at Portsmouth Regional Hospital opened in 1995, its clinical success with over 1500 patients has been impressive. 95% of the patients who come to the Portsmouth Center finish the program, and 92% of them leave healed based on outcome measurement data for patients who complete treatment.
